Log an incident
Recording what happened, and who is allowed to read it afterwards.
An incident is recorded against the child or staff member it concerns, with what happened, when, and who recorded it.
Who can read it afterwards depends on the role. Incidents are not visible to every member of staff simply because they are in the system — that scoping is the difference between a record people trust enough to write honestly in, and one they do not.
